THE AWARD
A Community Long Service Award is offered, in the form of a certificate and badge, to nurses on the NMC Register, in recognition of their contribution made to working as part of the NHS, for 21 years in the community.

HOW TO APPLY
You must apply with 5 years of retirement.
Send in application form by 31st September.
Certificates and badges will be presented annually in November.
