This August, one of QNIS’s Creating Connections projects will be presented to an international audience, as part of the 2015 ICCHNR Conference.
CHOICE, or Connecting Health and social care to Offer Individualised Care at End of Life, is a project seeking to develop and test an initial framework of integrated working to facilitate person centred care for patients and families at the end of their life who are being cared for at home. The project is led by Dr Caroline Dickson, a Senior Lecturer in Community Nursing at Queen Margaret University, and she has been selected by the ICCHNR to present on the project. The model used is still being developed, however the presentation covers the project while it is still being tested.,
The ICCHNR Conference takes place in Seoul, South Korea from the 19th to the 21st of August and this year you are able to live stream all the presentations live to your computer. Read our previous news post for details on how to attend from your own computer.
